Model Sign Up and Release

Let’s collaborate! Please fill in this form so that we can plan a shoot, it will also act as the model release form. You will receive a copy in your email if you provide one. Please double check your email and/or phone number as incorrect information will prevent me from replying to you.

 

The form will adapt to some of your selections but should only take 2 minutes to fill in. If you’re not sure just drop me a message.

The Shoot

Please give some guidance to the different shoot styles that you are comfortable with. 

Please note that this is non-committal and all ideas will be discussed ahead of the shoot and on the day.

All shoots are strictly professional, with no sexual activity between model and photographer.

Social Media

Please share your social media information to help grow our profiles and allow me to see you. If you don’t have social media or would rather not share them with me, please reply to the model release with some photos or whatsapp me.

Media Usage / Model Release

Please agree for the media produced as part of our collaboration to be shared as part of my portfolio and/or store.

If you have any specific requirements for the shoot, please outline them below. For example: No full frontal, no erect to be published online, video for private usage only, adult images for exclusive use by you as the model, etc.
